Because care and compliance are the same conversation.

Built for providers responsible for both care and compliance. Participant safety and regulatory compliance both depend on recognising risk before it escalates. PatternCare helps providers do exactly that.

Protect the people in your care, and the people who provide it.

The most serious harm rarely comes out of nowhere. The signs are usually already in your data. PatternCare surfaces them early, so your participants are protected, and so is your service.

"Behind every incident report is a person who can't always speak up for themselves. I've seen how easily the warning signs get missed, not through neglect, but because no manager can stay on top of that much at once. I built PatternCare so those signs are caught in time, and the people in care are protected."
TAHLIA ELLAM · FOUNDER

Most major incidents start as patterns no one connected.

The biggest NDIS penalties came down to the same thing: risks that were already sitting in the provider's own records. Tap any case to see what happened, and how PatternCare changes it.

474 reportable incidents filed late, some more than 300 days after the deadline. The Court found care failures alongside the reporting breaches.

In each case, the warning signs were already recorded, just never joined up. PatternCare exists to surface them while there's still time to act, for the provider and for the person at the centre of it.

Penalty figures and findings are drawn from public Federal Court of Australia decisions.

From incident report to actionable insight.

PatternCare sits on top of the systems you already use. It reads what your team logs, finds the risk building across it, and keeps the proof. Click through each step.

Step 01 · Capture

Reads straight from the systems you already use.

Every incident your team logs flows into PatternCare automatically. Nothing new to learn, and nothing slips through because someone forgot to copy it across.

No new system to learn. Your team keeps working exactly as they do today.

Step 02 · Detect

Grades every incident and finds the building pattern.

PatternCare scores each incident and connects the ones that belong together, surfacing the risk a busy team can't see across a fortnight of reports.

Graded against the NDIS Commission's own reportable-incident definitions, not guesswork. The same categories and timeframes you already report against.

Step 03 · Act

Your manager reviews, actions, and sets a follow-up.

PatternCare surfaces what a stretched manager would miss, then leaves the decision with them. Every call is recorded, so the handling trail builds itself.

If something serious is logged as routine, it's caught here, before it's missed.

Export an audit-ready pack in minutes.

The record builds as you go, so when the Commission asks, you don't spend weeks digging through logs. One export shows you stayed on top of risk the whole way through.

The honest answers.

I already record incidents in another system. Why do I need PatternCare?
No manager can remember every incident across every participant and still notice the same risk quietly recurring. There's simply too much going on. PatternCare reads every incident you record and connects the repeats automatically, so a building risk is clear early, instead of only after something goes wrong.
What happens after an incident is logged?
Each incident flows automatically from your existing incident system into PatternCare. There it's read, given a risk level, and checked against the participant's history for any recurring pattern. Anything that needs attention is surfaced to the right manager, who reviews it, decides the action, and sets a follow-up date if one is needed. That follow-up stays on their list until it's done and ticked off, so nothing logged is left unmanaged.
Is our information used to train AI models?
No. Your data is used to detect patterns for your service and nothing else. It's never used to train outside AI models, and identifying details are removed before any analysis. Your participants' information stays yours.
How does PatternCare help with auditing?
PatternCare keeps a complete record as you go, so you're not rebuilding history at audit time. Export an audit-ready report in minutes showing every incident, its risk level, and how it was handled, with proof you stayed on top of risk throughout.
Can I bring my past data in?
Yes. When you join, we can import up to five years of your existing incident records, so PatternCare detects patterns from day one instead of starting from a blank page. If your records go back further, we can usually bring those in too.
How quickly can we get started?
Quickly. We handle the setup with you and import your data, and most providers are up and running within a week.
Where is our data stored?
In Australia, always. PatternCare runs on Australian infrastructure in Sydney, and your data never leaves the country. We handle personal and health information in line with Australian privacy law, and identifying details are minimised before any analysis.
Is PatternCare live yet?
